Initial Evaluation
Choose this option if you have never been treated at Haven Pelvic or if your last appointment was more than 12 months ago.
Book Another Appointment
Choose this option if you are an established Haven Pelvic patient and have been seen within the past 12 months.
Medicare Patients
Please call before scheduling.
If you are using Medicare, do not book an appointment online. Please call our front office so we can schedule the correct appointment for you.

Out-of-Network Care
Haven Pelvic is an out-of-network physical therapy practice. Payment is collected directly by Haven.
Reservation Charge
Your card will be charged when you schedule: $250 for an initial evaluation or $200 for an established-patient appointment.
Cancellation Policy
A $100 cancellation fee applies when an appointment is canceled with less than 24 hours' notice.
